CCG will confirm your visit through an automated appointment reminder. You will receive a phone call two days before your scheduled appointment. Upon arrival, you will be asked to complete a patient information form, which will help us understand if you have any current medications and health problems. After your appointment, CCG will send this form to you, so please ensure that you correctly fill in your current address, phone number and insurance. A HIPAA acknowledgement form is also required.
Upon completion of your scheduled appointment, the doctor’s orders and any lab tests, procedures, or follow up visits will be reviewed with you by our front desk. Please make sure that you bring your insurance information as payment is expected on the day of your appointment.
